If you are required to file an appeal or want to request changes to your financial aid, select the appropriate form(s) from the links below.
Appeals
If you have received notification that an appeal is required, use the appropriate form below. To determine if you are required to submit an appeal, check your myÂé¶¹¹û¶³ account under the myFinances tab; Student Account/Bursar menu. Complete all forms in full, attach required documentation and sign. Incomplete, unsigned, or non-required forms will not be processed and will delay the processing of your aid.
Appeal Forms:
– Complete the Maximum Timeframe portion of this form if you have been notified you may exceed the maximum credit limit.
– Complete this form if you have a Developmental Credit Limit hold on your Financial Aid account.
- If your financial aid status is "suspended", complete the Satisfactory Academic Progress portion of this form to explain circumstances that prevented you from meeting Satisfactory Academic Progress Standards.
Requests
The following forms are used to request changes or updates to your financial aid. Please read form descriptions to determine if you are using the correct form and if they apply to your circumstances. Be sure to read all form instructions and select the correct academic year form before submitting. If you are unsure you are submitting the appropriate form, please contact Financial Aid for assistance.
Request Forms:
- This form is required if you are requesting that your cost of attendance be increased due to child (under age 13) or dependent care expenses.
- If you are concurrently enrolled at both Âé¶¹¹û¶³ CC and another college/university and would like credits combined for financial aid eligibility, use this form.
- Use this form to request a waiver to one of the requirements for the Oregon Grants.
- Use this form to request a Pell Grant adjustment if an institutional error prevented enrollment in the correct number of credits at the time a Pell Grant was calculated for the term.
- If you're completing prerequisites to apply for admission to limited enrollment programs, use this form to be considered for preparatory loan funding. An information sheet is attached to the request to help you determine if this is the right option for you.
- Parent(s) may complete this form if they are refusing to provide required FAFSA information.
– Use this form for miscellaneous requests to Financial Aid, such as enrollment changes, cost of attendance changes, or accepting previously declined loans.
– If you're a dependent student and want to request to be independent, use this form. See Dependent vs. Independent to determine if you may be eligible.
- Complete this form if you would like to increase your student budget for the purchase of a computer.
- Use this form if there has been a significant change in financial information since you filed the FAFSA.
- Use this form if there has been a significant change in parent(s) financial information since you filed the FAFSA.
- Complete this form to request an offer of the Federal Direct Unsubsidized Stafford Loan. Submit ONLY if you have already been offered for 2025-2026.
